On this page
The reviewed monthly compounded tirzepatide offers here range from $169 to $339 for medication plus required program charges, with dose-specific and account-only qualifications. Key prices and billing conditions were rechecked on official provider pages October 15, 2026. Longer prepaid plans can require substantially more cash upfront; these are advertised offers, not guaranteed checkout totals or the entire market.
“All-in” is useful only when it includes the required care or membership payment as well as the drug. It still needs a second question: what must you actually pay today, and what can change later? Our sortable compounded tirzepatide comparison separates recurring charges, first payment, commitment and estimated first-12-week cash so those numbers are not confused.
Monthly program prices: compare dose categories and billing
The following table summarizes selected monthly options, not a clinical ranking. Dose labels reproduce provider pricing categories; matching a category does not establish interchangeable products or instructions for selecting a dose.
| Provider and published price category | Required recurring charges | Listed combined price |
|---|---|---|
| Gentle Health, 2.5–5 mg/week | One medication-and-care fee | $169/month |
| Gentle Health, >5–10 mg/week | One medication-and-care fee | $219/month |
| Gentle Health, >10–15 mg/week | One medication-and-care fee | $269/month |
| Mochi Health, representative 2.2 mg/week example | $79 membership + $90 medication | $169/month |
| Mochi Health, representative 6.6 mg/week example | $79 membership + $144 medication | $223/month |
| Mochi Health, representative 11.24 mg/week example | $79 membership + $184 medication | $263/month |
| OrderlyMeds, monthly refill option | One program fee, charged per requested refill | $299/refill, described as one month |
| DirectCare AI, monthly option | Medication, clinician care, coaching and shipping | $339/billing cycle |
Sources: Gentle Health pricing, Mochi’s per-dose explanation and FAQ, OrderlyMeds pricing, and DirectCare AI plans. Mochi calls medication prices representative; the final prescribed-dose and pharmacy price appears in the account. This table uses its standard $79 monthly membership, not an assumed promotion.
An identical starting dollar amount is not enough to declare two offers equivalent. Gentle Health’s $169 tier and Mochi’s $169 illustration use different published dose labels and billing structures. Orderly’s monthly option also has customer-category qualifications. Use the detailed comparison’s product and commitment fields rather than sorting the headline column alone.
What might the first 12 weeks cost?
For ordinary monthly plans, three payments are a budgeting illustration—not verification that three calendar-month bills equal an exact 84-day supply. The examples below hold the provider’s published price category constant. They are not a proposed dose-escalation schedule.
| Option | Cash calculation | First-12-week figure |
|---|---|---|
| Gentle Health starting / medium / high tier | Three monthly charges at $169 / $219 / $269 | $507 / $657 / $807 estimates |
| Mochi standard monthly membership, low / middle / high published examples | Three charges at $169 / $223 / $263 | $507 / $669 / $789 estimates |
| Orderly new-customer starter | $299 two-month program + one $299 refill | $598 estimate |
| Orderly transfer program | $449 two-month program + one $299 refill | $748 estimate |
| Orderly monthly refill option | Three $299 charges | $897 estimate |
| DirectCare AI monthly option | Three $339 charges | $1,017 estimate |
| DirectCare AI named 12-week prepaid option | Full $927 payment upfront | $927 listed plan charge |
Except where a provider expressly states no added tax, these figures exclude any unresolved checkout tax and unpriced outside services. Orderly’s starter and transfer offers are for new customers and have different prescription-history requirements. Do not assume either discounted entry price is available to every shopper or renews unchanged.
Upfront payment is not the monthly equivalent
DirectCare AI illustrates the distinction particularly clearly. Its public page shows a 12-week plan at $309 per month but bills $927 upfront. The 24-week option bills $1,734 upfront and the 52-week option $3,497 upfront. Those are the cash commitments displayed, not permission to pay only one advertised monthly-equivalent amount.
A first-12-week cash comparison must count a required longer prepayment in full. It would be misleading to count only three months of a prepaid annual purchase and hide the remaining money due at signup. Conversely, an upfront amount covering more than 12 weeks should not be presented as though it buys only 12 weeks. Show both payment and coverage period.
Mochi also offers prepaid membership terms while billing medication separately. The membership discount changes one component, not necessarily the medication price. A prepaid membership plus ongoing medication bills needs its own cash calculation; dividing the membership across months is not the same as the amount withdrawn today.
What the listed price includes—and what remains uncertain
Gentle Health states that medication, doctor care, injection supplies when needed and refrigerated shipping share one monthly fee, without added sales tax. Its terms require two monthly payments before cancellation and allow cancellation 30 days after a refill request. Current-month charges are not prorated or refunded; shipment-related refunds are case-by-case. A lower tier should not be compared as though it is an unrestricted one-month trial.
Orderly lists consultations, messaging, medicine, supplies and shipping within its program charge. Its public pricing and terms do not settle every checkout tax or automatic-refill question. Mochi charges medication separately from membership; its FAQ says there are no extra tax or shipping charges, while its Terms of Use, section 4 allow applicable taxes, shipping charges and fees on medication orders. That conflict needs a checkout confirmation. The Patient Agreement on the same page allows a fee of up to $75 for an appointment missed without 24 hours’ notice, not a mandatory charge for every month. DirectCare AI includes supplies and shipping and separately advertises an optional blood-panel price. An optional service is not silently added to every plan, but a required unpriced service would prevent calling the result a final total.
FDA context is separate from price
The following FDA context was checked October 18, 2026; the price-check dates above have not changed. FDA states that compounded medicines are not approved products. Its unapproved GLP-1 concerns include adverse-event reports involving compounded tirzepatide or semaglutide prescribed above approved-label doses or with faster increases. Reports do not by themselves establish causation or comparative risk. This guide provides no dose or treatment recommendation.
Following the temporary shortage-related enforcement periods, FDA’s April 1, 2026 clarification describes patient-specific conditions and restrictions on essentially copying commercially available drugs under section 503A, plus separate bulk-ingredient conditions under section 503B. Neither an additive nor a price listing establishes that a particular prescription meets those conditions. See our compounding-framework explanation.
FDA’s March 3, 2026 announcement of 30 telehealth warning letters describes concerns about implied equivalence to approved products and unclear sourcing—not a finding about every company in this table. Our tirzepatide FDA-status article explains why a compounded offer is not an approved generic. This price comparison does not assess an individual product’s clinical suitability or a pharmacy’s compliance.
A short checklist before payment
Ask for the exact formulation and prescribed price category, the full amount due now, every recurring care and medication charge, the refill period, shipping and taxes, any required testing, and the earliest cancellation date. Confirm what happens financially if no prescription is issued or an order has already reached the pharmacy.
Then compare the same payment horizon rather than the smallest visible number. Our provider profiles for Gentle Health, Mochi Health, OrderlyMeds and DirectCare AI supply broader service context. The tirzepatide guide adds availability links. None of those listings guarantees a prescription, a particular price tier or service in every state.
The useful answer is therefore a dated price plus its conditions: selected monthly offers span $169–$339, while first-12-week spending depends on membership, dose category, entry offer and prepayment. Keep unknowns visible instead of labeling an incomplete checkout estimate “all-in.”
