South Dakota has Allara Complete Care access, covering medical and nutrition appointments through scheduled video visits. That program supplies the table’s full-service row; it is not just a nutrition subscription.
FOLX’s service-by-state table also contains useful, narrower information: it marks estrogen care available in South Dakota and testosterone care unavailable. Its combined gender-affirming hormone-care row would overstate that scope. Neither offering should be treated as a substitute for the other.
Midi’s separate perimenopause and menopause visits
For South Dakota, Midi Health is the per-visit option. Its menopause page advertises visits in all 50 states and offers a self-pay route alongside insurance. Read that as where Midi says the visit is sold: it is not evidence of an open appointment, of the credential of whoever is assigned, or of participation by any specific insurance plan.
Separate access from charges
The FOLX membership FAQ distinguishes full-platform access from medical charges: visits, laboratory work and medications are extra. Without full membership, Member Operations arranges limited-access visits; platform booking and care-team messaging are absent. Allara’s cash-pay explanation excludes labs, imaging, prescriptions and supplements from its subscription.
Medicaid and Medicare cannot pay for FOLX services. A Medicare patient can receive care under its no-claims agreement, covering claims by the patient and by FOLX.
