FOLX Health offers healthcare services designed for LGBTQIA+ adults. Its public menu includes gender-affirming care, primary care, mental health services, therapy, sexual and reproductive health, and fertility/family-building support. Full platform membership is separate from the cost of appointments, labs and prescriptions.
History and ownership
The company’s story names A.G. Breitenstein as founder and dates its public launch to December 2020. This is a launch date, not a verified date of incorporation.
FOLX announced a $30 million Series B on October 12, 2022, led by 7wireVentures. The release named Foresite Capital, Bessemer Venture Partners, Define Ventures and Polaris Partners as participating investors and described its direct-to-employer offering. It does not establish those investors’ current ownership percentages.
The terms, updated April 20, 2026, identify FOLX Health, Inc. as the platform operator and distinguish it from the affiliated clinical practices and individual providers. The platform states that it is not itself the healthcare provider.
Services and access
Members can book appointments and use the portal for messaging, navigation and referrals. The membership guide also describes community resources and peer-led groups. These features are not a promise of unlimited included clinical visits.
The state service table, updated August 27, 2026, separates estrogen services, testosterone services, primary/mental healthcare, talk therapy, couples therapy and ADHD services. For example, it marks testosterone services unavailable in Alabama and talk therapy unavailable in Alaska. “All 50 states” does not mean every service is offered everywhere.
FOLX’s Florida requirements guide describes a virtual consultation followed by an in-person physician appointment to sign consent for its gender-affirming hormone services. It lists a separate $50 in-person charge, without insurance billing. The guide’s body is dated August 27, 2024; confirm the current booking requirement with FOLX. This describes its published process, not the present legal status of Florida restrictions or a requirement for every FOLX service.
The current employers page offers workplace benefits alongside consumer enrollment. Employer coverage and the amount an employee pays depend on the purchased benefit, not just the retail membership table below.
Membership and clinical prices
The July 2026 billing guide and current cash price list describe these separate charges:

The annual membership works out to approximately $29.08 per month, not a $29 monthly installment. One paid monthly membership period plus a $159 initial visit totals $202.99 before labs and medication. With annual prepayment, membership plus that visit totals $508. These are arithmetic examples, not complete treatment quotes.
First-time monthly members are offered a 10-day membership trial; the billing guide says annual membership does not include that trial. It does not make clinical services free during the trial. Membership renews unless cancelled.
There is also a limited-access option without full membership, described in the company’s membership article: visits can be arranged through member operations, but the route excludes full-platform benefits such as online booking and care-team messaging. The reviewed pages do not establish a separate complete price schedule for that route.
Using insurance
The insurance guide, updated September 1, 2026, lists coverage by insurer, state and service. It says Medicare and Medicaid are not accepted. Some plans also cover membership, but that should not be assumed for every in-network patient.
Copays, coinsurance and deductibles can apply. With multiple plans, FOLX says it cannot bill secondary insurance as though it were primary. Laboratory and pharmacy coverage should be checked separately from visit coverage.
Cancellation and refunds
Under the membership cancellation guide, support-assisted cancellations require at least 72 hours’ notice before billing. Annual members may need assistance because the self-service cancellation button appears only in the final six weeks before renewal.
Paid membership fees are not refunded, including unused portions of an annual term, under the payment-cadence policy. Access continues to the end of the paid period. Later bills can still arrive for completed labs or insurance-adjusted visits.
The appointment policy is separate:
- More than 24 hours ahead: refund less a $3–$6 processing charge.
- Less than 24 hours ahead: $35 for a 15-minute appointment or $65 for a 30–55-minute appointment.
- No-shows, specified late arrivals and being outside the clinician’s licensed state can trigger the corresponding fee.
- Florida in-person appointments have a stated no-fee cancellation exception; FOLX-cancelled appointments receive a full refund.
Confirm the exact 24-hour boundary with the company. Cancelling membership does not automatically resolve future appointments or previously incurred charges.
Privacy and remaining checks
FOLX was among the recipients addressed in July 20, 2023 general OCR/FTC warning letters about website and app tracking risks. The correspondence urges review of potential health-information disclosures; it is not a company-specific enforcement action or a finding that FOLX violated privacy law.
The platform privacy policy, dated June 3, 2025, and the clinical privacy notice, updated February 6, 2026, describe different information-handling roles. They are published policies, not a compliance assessment by Inside Telehealth.
Before paying, verify the requested service in the patient’s state, the specific plan and membership benefit, the chosen billing cadence, and the total expected appointment, medication and laboratory charges.
