State access guide · Selected providers

Primary care in Colorado.

Company statements checked between September 10, 2026 and September 15, 2026: 4 covered companies — Circle Medical, FOLX Health, MDLIVE and PlushCare — advertise online primary care services to patients in Colorado. Confirm eligibility and total charges with the provider.

Four selected online primary-care options with service-level evidence for Colorado, separating membership, visit charges and insurance checks.

For patients in Colorado. These are advertised online services, not verified local offices, appointment slots or prescription guarantees.

Colorado access has a Medicaid exception

FOLX marks Colorado available for Primary & Mental Health Care in its state table, but its Medicaid/Medicare FAQ adds an important limit: people enrolled in Colorado Medicaid cannot receive FOLX clinical services. They may join for its referral network and community platform; that membership is not access to a clinical visit.

For other eligible readers, this selection concerns adult primary-care appointments, not every FOLX service. Confirm your location and the clinician’s role; neither the table nor this guide guarantees an appointment or medication.

Booking and payment

FOLX’s full membership includes online booking and care-team messaging. Limited-access visits use Member Operations instead and omit those platform benefits; request a route-specific quote.

Visits, medication and labs are separate from membership. Check the insurance guidance for your plan. FOLX does not accept Medicaid or Medicare for service payment; Medicare enrollees may receive care under a no-claims agreement, a different restriction from Colorado’s Medicaid exclusion.

PlushCare does not take Medicaid either

PlushCare’s primary-care page states that patients in all 50 states can book appointments with a board-certified physician, Colorado included, and on the same page that Medicaid is not currently accepted. Anyone covered by Health First Colorado would therefore be paying another way.

That other way is a $19.99 monthly membership after a 30-day free period plus a separate visit charge, listed at $129 for self-pay patients. The all-states line is the company’s description of who may book, not evidence of a Colorado office or an available appointment.

See the FOLX and PlushCare profiles and the national primary-care guide for broader context. Neither is a local-office listing.

Selected providers

Which providers serve Colorado?

4 companies with published profiles and service-specific source records. Listed alphabetically, not ranked. This is selected coverage—not every provider in the market.

Formats describe the program, not a guaranteed first-visit option in every state. A messaging or questionnaire option does not promise an initial evaluation without a live appointment. Read each provider’s service and state access notes.

Circle Medical

Virtual-first primary and preventive care for adults, booked as scheduled video visits, with annual wellness visits included and appointment-only in-person access for selected services

Official website ↗
  • Live visit

Physicians, nurse practitioners and physician assistants. The terms of use identify Circle Medical Care of California as an independent medical practice, separate from Circle Medical Technologies, which operates the website and platform.

Access channel: Direct to consumer

Cash pay · Insurance

A standard primary-care or wellness video visit is $120 self-pay; the equivalent in-person annual wellness or standard appointment is $200. There is no membership or annual fee. The company reports that insured patients paid an average of $35 per appointment over the prior six months, which is a company-reported average and not a guaranteed price. A $50 fee applies to appointments cancelled less than 24 hours ahead and to no-shows.

Colorado is one of the jurisdictions Circle Medical enumerates for video visits; the listing says nothing about services other than online primary care. Expect app-booked scheduled video appointments in a virtual-first adult practice, $120 for a standard self-pay visit and no membership fee, with an insured average the company reports at $35 rather than guarantees. Medicaid is not accepted, Medicare only in some states and subject to plan, and a missed or late-cancelled appointment is charged $50.

Circle Medical's online-doctors page says "See a Circle Medical provider over video in any of the following states" and links thirty-two jurisdictions: Arizona, California, Colorado, Connecticut, the District of Columbia, Florida, Georgia, Illinois, Iowa, Kentucky, Maine, Maryland, Massachusetts, Michigan, Minnesota, Nevada, New Jersey, New Mexico, New York, North Carolina, Ohio, Oklahoma, Oregon, Pennsylvania, South Carolina, Tennessee, Texas, Utah, Vermont, Virginia, Washington and Wisconsin. The page calls that list thirty-two states; it is thirty-one states and the District of Columbia. This covers Circle Medical's online primary care only. Circle Medical advertises other services but publishes no service-by-state list in this source, so nothing beyond primary care follows from it. Being able to see a provider over video in one of these places does not establish an office there, an available appointment, insurance participation, or that any particular treatment or prescription is offered.

State evidence checked · State availability source

The practice does not see patients under 18, and a clinician must be licensed where the patient is physically located. Both the practice and the individual clinician need to be in network, Medicare is accepted only in a subset of states and subject to plan, and Medicaid or Medi-Cal eligibility currently rules out care. Listed in-person locations are appointment-only for established patients, not walk-in clinics. Company-reported.

FOLX Health

FOLX adult primary-care visits, with full-platform membership or a separately arranged limited-access booking route.

Official website ↗
  • Live visit
  • Messaging or questionnaire

The primary-care page describes choosing a clinician for a scheduled virtual visit and messaging the care team through full membership. Check the assigned clinician's professional credential rather than assuming every visit is with a physician.

Cash pay · Insurance

The cash list puts a 30-minute initial primary-care visit at $159 and a 15-minute follow-up at $79. Full membership is separate: $43.99 monthly or $349 paid annually. Visits, labs and medication are not included in that membership fee. The limited-access route omits full-platform benefits; the reviewed page does not give a separate complete price schedule for it.

FOLX's state-service table lists its Primary & Mental Health Care column as available in Colorado. This row covers adult primary-care visits, not every separately listed FOLX service or medication. FOLX separately states that a person enrolled in Colorado Medicaid cannot receive its clinical services, though signing up for its referral network and community platform remains possible.

The company state table lists its combined primary and mental health care service as available here. Read from the company own state-by-service table rather than a nationwide statement. It does not establish patient eligibility, clinician availability, insurance coverage, or that any particular medication is offered.

State evidence checked · State availability source

The service is presented for adults and designed around LGBTQIA+ care. Its state table separately lists primary/mental health care in all 50 states and DC; that is not a guarantee for other FOLX services or a particular prescription. Insurance participation varies by plan and service. FOLX says it does not accept Medicare or Medicaid to pay for its own services and is not a Medicare provider, but its Medicare/Medicaid FAQ states that a Medicare-enrolled person may still receive care if they agree that neither they nor FOLX will bill Medicare for it. The same FAQ says a person on Medicaid in Kentucky or Colorado cannot receive clinical services through FOLX, though they may still join for the referral network and community platform; that is the company's policy tied to Medicaid enrollment, not a state law and not a withdrawal from either state.

MDLIVE

Scheduled adult primary care by phone or video, covering wellness screenings, routine care and support for ongoing conditions

Official website ↗
  • Live visit

A primary-care doctor seen by appointment rather than on demand, so availability depends on provider schedules.

Access channel: Health plan

Insurance · Employer or plan benefit

The copay may be as low as $0, but only through a participating plan; MDLIVE states that not all plans include primary care benefits. No self-pay price is published for primary care, which is the notable exception to the self-pay options offered for some other MDLIVE services.

Colorado is covered by MDLIVE's all-fifty-states network claim, which is a statement about licensure rather than about who may book. Primary care is offered only through select health insurance plans, so a Colorado reader should check the plan before the provider directory. Phone-visit rules vary by state, and Colorado's own requirements govern that choice.

MDLIVE's primary-care page says MDLIVE is available anywhere in the United States and Puerto Rico, and that MDLIVE has a vast, nationwide network of board-certified, licensed doctors in all 50 states and Puerto Rico. The same page limits who can use the service: "Primary Care is only available through select health insurance plans", and it adds that because not all plans include primary care benefits, the easiest way to determine if MDLIVE Primary Care is included in your plan is to create a secure MDLIVE account. Being in a covered state therefore does not mean primary care is open to you; it depends on the participating plan. It also does not establish appointment availability, which visit mode is permitted where you are, what you would pay, or that any particular treatment or prescription will be provided.

State evidence checked · State availability source

This service is available only through select participating health plans and is for adults, so an MDLIVE account alone does not provide access. Phone-visit availability varies under state rules. A prescription is not guaranteed and the service does not prescribe controlled substances. Company-reported.

PlushCare

Membership-based virtual primary care with per-visit charges, including ongoing care and prescription management

Official website ↗
  • Live visit

The cited pages describe the nation's top doctors and refer to physicians and other medical providers, without establishing a credential for any particular visit. The company states its therapists do not prescribe and that it has no psychiatrists, which matters when primary care is used as the prescribing route for mental-health medication.

Access channel: Direct to consumer, Health plan

Cash pay · Insurance

Membership is advertised at $19.99 a month with a 30-day free period, and the membership and the visit are separate charges. Self-pay initial visits are listed at $129. The company says most patients with in-network insurance pay $30 or less, which is a company-reported typical figure rather than a quote. The terms state in capitals that payment of the membership fee is not a requirement to receive medical services.

PlushCare's primary-care page includes Colorado in its statement that patients in all 50 states can book an appointment with a board-certified physician. That is where the service is offered, not a Colorado office, a reserved appointment or a promise that every service the page lists suits your visit.

PlushCare's primary-care page states that patients in all 50 states can book appointments to discuss their health concerns with a board-certified physician, so a visit can be arranged from anywhere in the country. That sentence describes the company's service range in general and sits in the page's FAQ rather than in a state-by-state service table, so it says where the service is offered, not which clinician, appointment time or individually listed service is available to a particular patient. Access is membership-based with the visit charged separately, and the same page states that Medicaid is not currently accepted.

State evidence checked · State availability source

The company describes itself as in network with most major health plans, and its terms note some members receive services through employer coverage in which case membership fees may not apply. A separate limited-access route is documented for booking by phone without the membership tools, with its pricing not fully published. Cancelling a membership and being refunded for visits already taken are separate questions. Company-reported.

How companies enter this list

Each selected company has a published profile, a maintained service row and a separately reviewed match to current evidence for this state. A general national footprint is insufficient. The list is selective; an omitted provider is not necessarily unavailable. Inclusion does not establish clinical suitability, state licensure or a currently available appointment.

Smaller and larger providers use the same source requirements and alphabetical ordering. Visit each profile for company history and wider context, or suggest another provider.
