Among the services reviewed here, the difference between a structured programme and a repeat appointment decides both what is included and, for one of them, whether it can be bought at all.
Programme or repeat visits
A structured programme enrols someone in ongoing management of a named condition. Teladoc runs programmes for diabetes prevention, diabetes management and hypertension. These are the services most likely to include monitoring and coaching between appointments.
Repeat primary care is the alternative: seeing the same service again for the same condition, billed as ordinary visits. For the services here the cited pages describe no enrolment step and no monitoring programme, and continuity depends on booking the same clinician. That is what those pages document rather than a general property of repeat care.
For the programmes documented here, the distinction decides whether they can be bought at all. Teladoc’s own self-pay page states that its condition-management programmes are not available for self-pay, in the same sentence as primary care, and it describes no additional programme cost for eligible employer or plan members. So that route runs through a benefit, while repeat visits can be bought with cash. That is these companies’ arrangements, not a rule about the category.
What “ongoing” costs
A programme’s cost is usually invisible to the member because an employer or plan pays it, which makes it hard to compare against a cash service.
Repeat-visit care is easier to total and often larger than expected: a visit price multiplied by the number of times a condition needs review in a year, plus any membership, plus prescriptions filled at a pharmacy. A service advertising a low visit price and a low membership can still cost more annually than it first appears.
Where the evidence runs out
This guide reports what companies publish, and for chronic care several publish less than for their headline services. No company here publishes a price for a named chronic-condition programme: LifeMD publishes a membership price and separate visit charges, PlushCare publishes visit prices, and Teladoc publishes no member-facing price because an employer or plan pays for the programme, stating no additional cost for eligible members. Where a detail is absent it is recorded as absent rather than turned into a claim that the option does not exist.
Questions worth asking before enrolling
- Is this a programme with enrolment, or just repeated appointments?
- If it is a programme, can I get it without an employer or health plan?
- Is monitoring equipment included, and who pays for supplies?
- Will I see the same clinician each time?
- How many visits a year does this realistically involve, and what is the total?
- Are prescriptions billed separately at a pharmacy?
